1. Authorization of Agent form duly completed (must be filed within 2 months of application)

2. Name of Applicant (s)

3. Full address, telephone and fax numbers

4. Nationality

5. Country of residence or principal place of business

6. Whether the applicant is the creator of design

7. If the creator is not the applicant:

(a) name of creator
(b) address of creator
(c) a statement justifying the applicant's right

8. Four (4) graphic representations of each industrial design

9. Four (4) drawings or tracings

10. A specimen of the article embodying the industrial design where the industrial design is two dimensional (optional)

11. The kind of product for which each industrial design is to be used.

12. Whether the applicant requests publication of the Industrial Design be deferred and the number of months of deferment (maximum period of deferment is 12 months)

13. For joint applications, if the designs of the same application relate to the same class, the number of the class of the International Classification.

14. If priority is claimed, the country, filing date and application number of the earlier application. The certified copy of the earlier application must be furnished within three months of the filing of the present application form
